CLINICAL FACULTY ASSOCIATE MODEL PROGRAM
CFA STIPEND OPPORTUNITY

All Clinical Faculty Associates who are enrolled in the Teaching Nursing master's program, Teaching Nursing post-master's certificate program or taking continuing education classes in Teaching Nursing at the University of Utah will receive a $2,000 stipend each semester they meet eligibility requirements.

1. Eligibility
1. Recipients must be enrolled in the Teaching Nursing master's program, Teaching Nursing post-master's certificate program or taking Teaching Nursing classes for continuing education.



2. Recipients must be serving as a Clinical Faculty Associate for the College of Nursing during the semester the stipend is awarded.



2. Duration
   Funded though a High Growth Job Training Initiative grant from the US Department of Labor, these stipends are available on a per-semester basis through June 2008.



3. Agreement and Understanding
   Recipients must be a CFA during the semester the stipend is awarded, and enrolled in the Teaching Nursing master's program, Teaching Nursing post-master's certificate program or taking Teaching Nursing classes as continuing education. The stipend must be used as detailed in the financial section below (4). This form must be returned before payment can be processed. Stipends will arrive during the semester.



4. Financial
   This stipend is intended to support the CFA's enrollment in Teaching Nursing. As such, the stipend must be used for costs associated with the Teaching Nursing program including: computer, web camera, high speed internet service, non-reimbursed books, tuition and/or immunizations. I understand if I do not complete the course I am enrolled in I will reimburse the College of Nursing for the unused portion of the stipend.
